Why enrol with us?

Enrolling with Stortford Accident & Medical means you’ll pay lower fees, receive publicly funded healthcare, and gain access to services through our PHO. It also allows us to keep your medical records up to date, making it easier to support your long-term health.

Enrolled patients benefit from more consistent, personalised care. You’re more likely to see the same GP or nurse, and having your health history in one place means we can offer advice and treatment that reflects the full picture.

Who can enrol?

To enrol, you must be eligible for publicly funded healthcare in New Zealand. Most citizens, permanent residents, and some work visa holders qualify. You can check your eligibility on the Health New Zealand website.

You can enrol children under 16 in your care by completing a separate form for each child. If a family member is 16 or older, they’ll need to enrol themselves unless you have Power of Attorney or are their authorised representative.

What you’ll need

Before you start, make sure you have the following ready:

• A valid form of ID (passport, birth certificate, or visa)

• Address and contact information

• A few minutes to complete the online form

Proof of your identity can be any one of the accepted documents listed in the eligibility checklist.
